Continuing Care Nursing Team, Continuing Care

 

During a season of redevelopment for the continuing care program and the site at Killam Health Centre, there have been many significant changes to both the physical environment and to the model care. Every resident was reassessed and family/resident meetings were held with staff to review care plans and changes in care needs and care delivery. Preparing to move all 40 residents required an incredible amount of planning, coordination and collaboration with all the stakeholders. Communication was key to ensure that everyone was engaged and their needs were met. With the expected challenges that come with such as significant change, the team responded with support, positive attitudes, respect and professionalism.

The move to the new Campus of Care was significant, but it was just the beginning of changes to routines and to finding new ways to coordinate care to best meet the needs. Killam moved from being a long-term care facility to offering multi-level care, including SL4 and SL4D. This involved significant collaboration with Alberta Health Services case managers who manage the care of SL residents. With a myriad of new processes, forms, roles, faces and concepts, communication and collaboration have been key. Through it all, the team has grown stronger, they are fully engaged and have stayed focus on the goal of ensuring that care for the whole person — body, mind and soul — is upheld.
