Connect Care Implementation Team, Clinical Systems Support

The Connect Care Implementation team is a model of collaboration. This small, but mighty Covenant Health team worked incredibly hard to engage sites involved in Wave 1. 

The team members came from a variety of backgrounds and this was their greatest strength. Their previous frontline experience, coupled with leadership and managerial experience, allowed the team to work together to achieve a successful roll out. Each member of the team has a wealth of knowledge and a way of teaching new information that is supportive and inclusive of the unique needs of users.

In all, there were 82 distinct and diverse clinics, 135 super users and more than 1000 frontline users trained. As a team of four people, they could not be everywhere all the time. They did a tremendous job of engaging leaders (managers, super users) to train and empower them to do relevant work in their own areas, thus leveraging their own time and planning for ongoing success and support.

Connect Care implementation impacts every aspect of healthcare - the patient experience, front-line provider workflow, manager reporting, patient registration and health information management. This team used their skills in planning, organizing, networking, coaching, delegating and communicating to enable successful roll out.
