How to submit a nomination

Step 1Select the category

There are six categories to choose from as you prepare to nominate an individual or team for a Mission Award. The categories are our six values. The criteria used to review each nomination is based on how we define these values. Take a moment to review the criteria for each value, then select the category that best aligns with your nominee's qualities. Each value has its own online nomination form, so be sure to select the appropriate one when filling out the form.

Step 2: Complete the nomination form & gather or take 2-3 photos of your nominee in action.

To make the nomination process smoother and more efficient, you can now complete the form directly online. The form includes seven criteria statements that define the chosen value. Select three of the seven criteria and provide examples of how your nominee embodies each one. You may collaborate with other team members to strengthen the nomination.

You can save your progress at any time and return later to complete the nomination. You may also share the link with another person to contribute to the nomination. Once you have filled out the nomination form, you will be prompted to upload 2-3 photos of your nominee, along with the necessary photo consent forms for everyone appearing in the images. Please refer to photo guidelines for more information. After completing the form, send the link to the nominee or team lead for their review and digital signature.

Step 3: Submit the nomination form by December 13, 2024

All nominations must be submitted by December 13, 2024. Once you’ve completed the form and both the nominee/team lead and nominator have signed it, and you’ve uploaded the required photos and consent forms, simply press "Submit." Your nomination will be automatically sent to your designated site champion.

While online submission is preferred for efficiency and ease, if you prefer to handwrite your nomination, please contact your site champion to obtain a copy of the paper form. Should you have any questions about your nomination or encounter difficulties submitting the form online, your site champion is available to assist.
